Our Community Pathway is designed to give young people and young adults with SEND opportunities within the local community. The sessions aim to increase socialisation, build upon interests and give young people confidence to explore their local community with friends.
Whether you find forming friendships challenging or find that leaving the house is difficult, this pathway will support you with your personal goals by:
- Creating social groups that will plan your activities within the community (shopping, eating, bowling etc)
- Encouraging participation
- Supporting you to think about your interests (what you like to do and where you like to go)
- Tailored support for individuals
- Blended sessions for those unable to leave home
This pathway is open to disabled people who are:
- Aged between 6 and 25 years old (separated into age groups and locations)
- Looking to gain friendships and feel connected to your community
- Looking to increase your confidence and independence
Programme Information
These sessions offer safe and relaxed environments where new relationships and experiences can be explored. Our friendly team will listen to your desired outcomes to help support, encourage and direct a plan that will enable these to develop. You will be separated into three age groups with the idea of allowing participant-led activities, dependent upon interest, hobbies and individual needs.
The program will focus on building skills in areas such as:
- Developing hobbies and interests
- Confidence to start or maintain friendships
- Socialising skills
- Feel a part of the community
- Lead equal, rich and fulfilling lives
- Increase standard of living and wellbeing
